The National History Challenge is obliged to provide information about entries to its sponsors. This information will also help us to provide appropriate certificates for all participants. Certificates cannot be provided until this sheet is submitted.

Please assist us by filling in the form below and forwarding it to your State/Territory Co-ordinator, as well as the School Judging Information Sheet, when submitting your winning school entries.

Teachers should use their discretion and nominate students for participation certificates who have shown genuine effort and keen historical inquiry. Some students may not have completed their task but nevertheless deserve recognition. These students MUST be included in school statistics.
